The business entity, clinical governance, HIPAA posture, and supporting policies behind u-wellness and its IV service line. If anything here is unclear, legal@u-wellness.ai.
Business entity
u-wellness is the consumer wellness brand operated by:
Mojabi Labs, Inc. — a Delaware C-corporation
d/b/a Wellness Clinic Fresno
Contact: hello@u-wellness.ai
Clinical governance
Clinical services are delivered under a California Professional Corporation:
Wellness Clinic Fresno, P.C. — California Professional Corporation
Medical direction: California-licensed physician review before protocol approval
Clinical operations: Jenny Andersen, RN (California registered nurse; verify by name through the California Board of Registered Nursing lookup)
Independent lookup: California DCA license lookup
Every protocol is reviewed and signed by a California-licensed physician before it is prepared or administered. Every infusion is administered by a California-licensed registered nurse. We do not pre-mix, pre-bag, or administer IV therapy without physician sign-off on that individual patient’s protocol for that specific visit.
Compounding & supply
u-wellness does not compound IV ingredients in-house. Ingredients are sourced from licensed pharmacy suppliers and compounding pharmacies operating under state board of pharmacy oversight. Pharmacy details are shared with patients and clinicians as part of visit documentation or on request by emailing legal@u-wellness.ai.
HIPAA & data handling
u-wellness is a HIPAA-covered health service. We maintain administrative, physical, and technical safeguards for protected health information (PHI) consistent with the HIPAA Privacy and Security Rules.
What we collect: intake responses, medications, allergies, conditions, goals, uploaded lab results, visit notes, protocol history, payment records (not card numbers — Stripe handles those).
Who can see it: the physician reviewing your case, the nurse treating you, and ops staff who schedule your visit. Access is role-gated.
How long we keep it: medical records are retained per California and federal requirements (minimum 7 years for adults).
What we don’t do: we do not sell patient data, share it with marketers, or use PHI to train third-party AI models.
Subprocessors: we use standard infrastructure vendors (cloud hosting, email, SMS, payments) under data processing agreements. A current list is available on request.
Your rights: request a copy of your record, request corrections, or request deletion by emailing privacy@u-wellness.ai. We will respond within 30 days.
AI disclosure
u-wellness uses large language models (LLMs) to assist with patient intake, protocol drafting, and contraindication flagging. AI is a drafting and triage tool in our workflow. Every clinical decision — ingredient selection, dosing, and whether to proceed with treatment — is made by a licensed human physician. AI does not sign, approve, or release protocols on its own.
Telehealth disclosure
Clinical encounters with u-wellness are delivered in a combination of asynchronous review (the physician reviews your intake and labs without a live video call in most cases) and in-person administration (the nurse visits your home). If a live physician consultation is indicated, we will schedule a California-compliant telehealth encounter before proceeding.
Communications & SMS
u-wellness uses SMS only for outbound transactional messages tied to your account: payment links, approval notifications, booking links, appointment reminders, and post-visit check-ins. We do not operate an inbound SMS intake channel, and clinical intake is not collected by text message. We do not send marketing SMS without opt-in. Message and data rates may apply. Reply STOP to opt out and HELP for support where supported.
Payment & refunds
Payment is collected up front via Stripe (card, Apple Pay, Google Pay). Card details are handled by Stripe and are not stored on u-wellness servers. Cancellations more than 12 hours before a scheduled visit are refunded in full. Short-notice cancellations (< 12 hours) may incur a preparation deposit or rebooking fee — see Terms for current policy.
Regulatory notices
Not emergency care. u-wellness is not urgent care and is not equipped for medical emergencies. Call 911 for emergencies.
Not a substitute for your primary physician. IV therapy is an adjunct to, not a replacement for, standard medical care. Bring your u-wellness records to your primary care physician on request.
FDA / compounding. Compounded medications are not FDA-approved in the same way manufactured drugs are. See the FDA’s patient information on compounded drugs for details.
